Each player is counted once, by the rank Valve showed after their latest ranked match in the last 30 days. Players still in their calibration games have no rank yet and are not counted. "Higher than" is the share of counted players in a lower rank or sub-tier.
The numbers come from deadlock-api.com, which collects match data from the community, so they are a large sample and not Valve's own count. From Eternus I on, Valve sets the sub-tiers by daily percentiles, so the Eternus sub-tiers move from day to day.
